Transition Webinar: What is a Supports Coordinator? How do I get one?

Join the Include Me team for part one of a three part series on transition! This is a virtual webinar (Zoom) series where we will discuss transitioning from school to adulthood for people with disabilities. This webinar session will cover supports coordination and how it can help a person get access to essential services once they’ve graduated from school.
